2. Present application, appropriate fees, and photo ID at the Bollinger County Health Center

Or follow mailing instructions on the application.

• Birth Certificate - $15 per copy
• Death Certificate - $14, additional copy at same visit $11/each

If not born in Missouri

1. The Bollinger County Health Center cannot print birth or death certificates for people born outside of the state of Missouri.  Employees at the Health Center have contact information for other states or you may contact a Health Department in State of Birth for assistance printing birth or death certificates.
